You might try the tools at

   ftp://ftp.ncbi.nih.gov/pub/archive_dtd/tools/

which are pointed at from http://dtd.nlm.nih.gov/tools/tools.html

Specifically,

   ftp://ftp.ncbi.nih.gov/pub/archive_dtd/tools/ViewNLM-v2.3.zip

contains both an XSLT stylesheet and a CSS stylesheet (you
want both).

If I understand correctly, the tools just mentioned work
with versions prior to version 3 of the Archiving and
Interchange DTD for journal articles; stylesheets for
version 3 are
