We use the Barracuda Filter.  It takes a month or so to learn what you consider spam and what you don't.  At present maybe two or three spasm a month makes it into my mailbox out of 50 to 100 e-mails a day.  My only complaint about it is that occasionally it seems to think the GNLIB_L is spam.  Well, then you know how racy those graphic novels can be.
