I really like GFi MailEssentials (http://www.gfi.com/mes).  It
integrates with our Exchange server, configuration is simple and
straightforward, and it's worked great so far for us. It even blocks the
NDR spam that the built-in Exchange filters ignored, and the Bayesian
filtering is pretty good at learning what's spam and what isn't. False
positives are very rare.
