[Publib] Book A Librarian compiled responses

Hello friends,

A couple of weeks ago, I inquired about Book a Librarian services in which
patrons can make appointments with librarians for personalized, one-on-one
instruction. I got a great response, and I thank you all. Some people asked
that I compile the responses that I receive. I put together a document that
covers the prior discussions on publib and libref-l about this topic, the
responses and comments to my post, and an appendix with more information
about the responding libraries.
