[Publib] Survey questions

Our library will be conducting a telephone survey of our residents soon.
We aren't conducting this survey because we are contemplating major
changes (like a new building), but we are interested in what our
community thinks about our service and how we can serve them better. We
will be using a professional company to help us formulate the questions,
conduct the interviews, and tabulate the results and I'm sure that they
will have valuable suggestions. However, for those of you that have
conducted such a survey fairly recently, could you tell us what question
or questions you asked that gave you the most valuable or surprising
information, and what question or questions you didn't ask that you
thought "Darn, why didn't we think of that?" after the survey was
completed. Please post to me off list and I will compile the results if
there is interest from others.
