I would be interested in what people are using for online customer
service surveys as well. 
I know that this is one of the services Counting Opinions offers. 
We've used the paid version of SurveyMonkey for various things, but have
not done any kind of public library customer service survey. SurveyGizmo
is another build-it-yourself service. 

I promise, none of these outfits give me kickbacks. ;-)
