[Publib] Staff evaluation forms

Friends,

 

We are wanting to redo our evaluation form and are looking for great ideas
from you. We have collected some samples from various sources but now would
like to hear what you have that you think is great. If you have a form you
are using that really works and are willing to share a copy please forward
it to me. Staff that are evaluated are both professional and nonprofessional
as far as degrees. Currently we do a 3 month review then a 6 month and
yearly evaluation. After that everyone is evaluated every year. So pass on
to me those forms that work for you.
