[Publib] Family vs. individual responsibility

I'm re-visiting our Borrowing Policy and am curious how other
libraries handle individual vs. collective responsibility for checked
out items.

Specifically, say you have 2 parents and 2 kids, one age 8 and one age 16 --

If matters escalate to the point where borrowing privileges are
suspended, do you suspend for the one patron in question, or for the
entire family?

Does it matter whether the villain -- okay, that was a bit strong --
is one of the parents? the teen? the child?
