[Publib] Streamlining patron registration

We have a single page for library card registration, which we shred at
the end of each day.  Recently, we've piloted a program of laminating an
application, and using a dry erase marker, or simply taking the
information from a picture ID and asking for the remainder of the
information.  At the end of the transaction, the sheet is wiped clean
and reused.  In two months we've saved a ream of paper in just our
branch.

Computer usage guidelines appear as a pop up screen when they log into
the internet: accept and get on, decline and your session is ended.

We don't have anything like DVD/Video rules.

I'm having troubles posting to the list and hope this goes through.

We currently have 3 forms that new patrons sign - 1) registration with
name,
address, etc., 2) DVD/Video rules, 3) computer usage guidelines.    We
do
save the papers after all is entered in the computer thinking that
someday
we may have to prove they signed if there are problems.   Anyone have a
more
streamlined system like putting all on one sheet and needing only one
signature?   Do you save the papers?
