we run a similiar service as an outreach one. Customers are sent a list of genres to select that they would be interested in receiving titles from, and then staff at outreach and special needs services facility select titles which are sent to a 'home' library. Customers can either pick these up themselves, or nominate a 'courier' (a friend or family member) to come and collect them. If the customer has no-one who could pick them up, and are unable to get into the library, a staff member will drop the books off to their house. These are issued to a concession card which gives the customer 6 weeks, rather than the standard 4.

I am interested in starting a "Books by Mail" program at my library for those patrons who are physically incapable of coming into the library itself. I am interested in learning how other similar programs work and if there is a way to do this at low cost.
