[Publib] Having a materials challenge form online

I agree that the form should be available online but should have to be presented to a librarian for submission. A face to face encounter with a librarian is an important way to begin a materials challenge. The librarian might be able to answer any questions the patron has or the patron might be able to better describe their issue verbally than in writing.
