I agree with Andrea that having the conversation is useful for the library
and the patron, but here is an idea to add the convenience of an on-line
form and promote the library...  put the form behind a link using EZProxy or
some other proxy server software, so that in order to access it online they
must have a library card!

that way they have the option and if they really want to object they might
be encouraged to make sure their card is up-to-date...  and as always have
the in-person option available as well.
