[Publib] animal program consent form

I have an unusual request from the collective wisdom.  We have held
hamster race programs for the past 4 years, and they've been very
successful at bringing people into the library.  Unfortunately, last
year one of our little racers expired a week or so after the race.
Information from the owner was sketchy, but she implied that the hamster
caught "something" from one of the other critters.  They are required to
be in their racing balls at all times, but being rodents, there is the
occasional effluvia, if you get my drift...  Anyway, what I'm after is
some sort of consent form that absolves the library of any
responsibility for the animal involved in the activity.
