Several libraries in our area are using E*vanced 
<http://www.e-vancedsolutions.com/>. It allows staff and patrons to 
book rooms over the Internet, either remotely or in the library 
(http://tinyurl.com/yprug8). Staff reservations are automatically 
approved. Patron requests are routed for staff approval, though the 
time is blocked out. Setups can be specified, with furniture layout 
and equipment chosen from library specific lists. The resulting 
calendar can be loaded on your website 
(http://tinyurl.com/yprug8).  The software also generates email 
reminders of the reservation a day in advance.

Far from the only such product, but one that seems to meet your requirements.
