[Publib] Shelf List Card Template

[Miranda Burton, Outreach, HCMPL]

Do any of you out there in library land have a template that you use to 
make shelf list cards on your computer?
I can use the typewriter.... but would prefer to use computer.. 
unfortunatly I have not been able to line things up correctly using 
computer.
Any help is appreciated!
