[Publib] Refunds for lost paid items

We refund up to one year from time of payment, and individual must have
receipt or the item must still be listed in their payment history (our
ILS lists payments up to 1 year) If borrower returns item we refund
cost minus whatever late fee is due (our maximum is $10.00). If we
locate on shelf, we refund total amount paid. Refunds are mailed by
check mailed to the cardholder to which the item was checked out. No
refunds are maid for items older than 1 year.

We are currently examining our refund policy and would like to hear what
other systems do. As it is now we do not have a threshold on the price
of
the item and allow a refund up to six months from the date paid. The
refund
is issued as a check and mailed from our finance agency.
Do you allow refunds? If no, how did you come to that decision?
For what time period after something is declared lost or paid for do you
allow a user to get a refund?
Do you have a threshold on the amount for which a user can get a refund
(minimum or maximum)?
Anything else you can add would be welcome. Replies welcome off-list or
on.
