[Publib] New Patron Information

Whoops - almost forgot - we also require two forms of ID (for children
we ask whoever is guaranteeing the card to provide ID). One of these
needs to preferably by photo ID. If a customer presents their passport
we will accept this, and do not require the 2nd form.
