[Publib] New Patron Information

We ask for full name, DOB, postal address and residential (if different
from postal), phone number and email (if applicable)
 
We ask for a second contact address as well, and tell them that they are
required to inform their 2nd contact that they have been put as a
contact at the library. Most people use family members for this. 
 
We require a guarantor for all children under 16 who are applying for
library cards - this person is then responsible for any debts/charges
incurred by the child.
 
To cut down on people giving us false details new borrowers are required
to show us 'proof of address' - which needs to be an official letter
(bank statement, power/phone bill, etc) from the last 30 days with their
name and address on it. Until we have sited this borrowers are
restricted to 2 items able to be issued. Once seen, this then becomes 20
items issued.
