We require date of birth, permanent address (for college students living
in dorms and visitor cards), email address isn't required but we have a
space for it and they can get notices, etc. that way if they choose. We
ask for their driver's license #, if possible. For children, we
additionally require the name of the "guardian" who is responsible and
must apply for the card.
 
The date of birth is most critical, since it allows us to tell the
difference between all the Mary Smiths and whatnot and helps us identify
women (generally) who've changed their last name and already have a
card. We can search by almost any field, so we do a birthdate check on
new registrants to catch those.

and phone number do you ask for?  Does it vary between adult cards and
children cards?
