[Publib] IT User Satisfaction Survey

For various reasons, my director feels it might benefit our City's IT department to conduct a User Satisfaction Survey for its internal customers (i.e., City workers).  Does anyone in PUBLIB-land have access to such a survey?  Or perhaps you have participated in one recently?  We would love to have samples, but any suggestions on content would also be appreciated.  Please feel free to send the information to me off-list.  I will be happy to share results with anyone who asks.
