[Publib] Database evaluation forms

Greetings, colleagues!  We use an evaluation form for databases we are considering for purchase.  It works OK, but some of our evaluators find it confusing.  Does anyone out there have a form they can share?  Even if you don't think your form is perfect, it might be useful for ideas.

Since publib doesn't do attachments, responses with attachments should come directly to me.
