[Publib] patron challenges

Hello All,

My library is in the process of reviewing our policies and procedures
regarding patron challenges to materials and I would sincerely
appreciate your help. One of our focuses is reviewing and improving
training for staff who handle patron complaints in branches.   

We are seeking to collect the following from other library systems: 


*	Copy of Library's procedure starting at the branch for handling
patron complaints
*	Copy of Library's policy and / or supporting documents regarding
intellectual freedom and challenges
*	Copy of Library's patron complaint / request for reconsideration
form
*	Sample letter to patron (if applicable, and with confidential
elements removed)
