[Publib] "Damage" labels on DVD or CD cases

We do a "problem slip" that we put inside each DVD and CD.  There are several options the 
patron can check or write in specific problems.  Some patrons are very specific and write 
down track numbers, etc. which is very helpful.  Some patrons just write in "doesn't work" or 
"doesn't play."  We ask on the slip that patrons either place the slip underneath the sleeve on 
the item or call it to the attention of staff when they return items.  We replace the slip when 
necessary, and that way it doesn't cover up any "important details."

It seems to work fairly well.

I like the idea of the damage disclaimer.  However, doesn't this imply that the library's items 
might be capable of inflicting damage?  Seems to me there was some legal implication....
BUT I am not a lawyer (nor do I play one on TV) so this may not be true.
