[Publib] banned patrons

Kelley,
The few times I've had to ban someone they were notified by letter sent
registered mail. In the case of the minors, we got the parents name and
address from the school- they were not library card holders. I also
notified the police station so if they were called, they were already
aware. The two kids were ongoing challenges so the police were very well
acquainted with them. I also issued a memo to staff with the name, date
and length of the ban.

We haven't had to ban anyone from the library permanently (although we
did get a restraining order against an idiot who was harassing staff,
keying the director's car, and bragging about it)/  We have rounded some
kids for 30 days, meaning they can't come into the library without an
adult to supervise them.
 
If someone is to be barred from the library for a certain time, and s/he
is a kid we will send the parent or guardian (whose name we have from
their librar card registration) a letter, including the reasons thhe kid
is being grounded.

have had to ban patrons under the age of 18 let the parents/guardians
know that their kids have been banned from the premises? If so, do you
call, send a formal letter? Also do libraries write something up every
time they ban a patron? If so, how long do you keep these forms? If
anyone would like to share their incident report forms or procedures
