[Publib] "Memorial" form

Does any library out there have a form that people fill in to give
donations of money for books to be added to the collection?  We have a
form that people use to give money and ask us to buy items in memory of
a deceased relative or friend.  However, the form's as old as the hills
(or probably older) and I'm trying to revise it.

I'd appreciate any help you can give.
