At my library we have a $10 total fine cap, excluding lost items-- 
e.g. if you have $50 in fines and a lost book worth $20, you have to  
pay a total of $30; if you have $50 in fines and no lost books, you  
have to pay $10.

I suspect this policy works, to the extent that it does, because we  
don't lend DVDs, videos, or music CDs, and because the library system  
is large enough that we never order just one copy of a book; if it's  
worth ordering, there will be at least 6 in the system, so books  
usually get to people in a reasonable amount of time even without  
intimidating fines.
