Does anyone out there in library-land have a certain format that you use in
making an invoice to patrons to items lost (or in this case lost in a fire)
for them to turn into their insurance company (or whomever needs to know)
I generally just whip one up in Word but I would like to have a set format
to just input the information and mail it out.
