[Publib] Netlibrary user survey

We started a subscription to Net Library's Recorded Books (downloadable
e-audio and e-books) one year ago.  I would like to do a user survey to
determine the satisfaction levels of the service. Has anyone done a user
survey? I'd like to see samples...
