[Publib] Acquiring fillable legal forms for free?

Hi,
  I work in a small public library and am trying to help a patron find a fillable legal form (one that he can download and change the text of). He specifically needs a "restraining notice to judgment debtor" for New York State. It is form CPLR 5222. I can find the form online, but for a fee. We would like one that is free.
