We charge the cost of the item plus a $10.00 Processing fee for each item. If the customer finds the exact same item in good shape, s/he can purchase that to replace the missing item, but they're still charged the processing fee.

Subject: [Publib] RE: Lost items
<<Could anyone please share your replacement policy for items not
returned by patrons? Do you charge a replacement or processing fee in
addition to cost of book? If book is out of print do you adjust the
cost? I appreciate any information that you can share.>>
We charge the replacement cost of the item plus a $7.50 processing fee
($15.00 for audio books) for any items not returned. In the case of a
lost part of any item, we charge a flat replacement charge based on the
item type. For out of print items, there is an average price we use,
again depending on the item type. For example, an out-of-print adult
nonfiction book which is not returned is charged to the patron at $30.95
plus $7.50 processing fee.
If the item is returned after being set to a "lost" status, the patron
is credited only for the amount of the item; the processing fee stays on
their account.
