[Publib] Speaker Confirmation Agreement

I am trying to create an agreement to be used with all speakers/authors we
book at the library.  The goal is to have an agreement signed by the library
and the speaker that outlines the details for a particular program: date,
place, time, outline, maps to library, AV needs, etc.  Then both the library
and the speaker have all necessary information before the event. Does anyone
have a document I could look at?
