[Publib] Staff Development Day Evaluation

I am the chair of my library system's Staff Development Day Committee.
One of the things I have been tasked with doing is creating a program
evaluation form. I would like to design an evaluation form that is
handed out at the conclusion of the program and turned in before the
attendees leave. The idea behind this is to capture thoughts and
feelings while things are still fresh in their minds and before they get
back to the branches and their regular responsibilities. We have also
opted for this because it's easier to be anonymous with turning in a
form than sending an e-mail and it's been my experience that some people
are more willing to provide feedback when they know that they won't be
identified with it.   I have a basic idea of what kinds of questions
should probably be included, but I want to make sure we ask the most
appropriate questions for the type of information we want to collect. I
am hoping that the feedback we receive will help guide the planning of
future staff development days. If you have an evaluation form from a
Staff Development Day that you would be willing to share, I would very
much appreciate it. If you have suggestions of questions to consider
including on the form, that would be helpful as well. Thank you for your
