I agree with Poul. It's very handy to put the blame on others but it may backfire on you and your ability to fill requests. Customers have been known to call lending libraries. For example, customers from a certain library in north Georgia have called us (usually I get the calls) berating us (I'm understating this) for not filling requests for photocopies from the Florida Times-Union. When I check our records for the requests, I find we never got the requests. I've had one of this library's customers call me a liar when I told her we didn't receive the request. Someone dropping the ball and then either their ILL staff or their reference staff is blaming it on us. I don't enjoy these exchanges -to the point that I'm considering blocking the library. We are one of the few free suppliers of photocopies from this newspaper.

Why would you want to not tell the truth and instead act as if this is a policy imposed by other libraries? We don't care how many times we lend a book to another library and if it's going to different users or to the same person is of no importance to us.
I think you need to tell your users if you have a problem and be honest about what the problem is and why it's such a big deal.

I just recently came across that situation myself. My supervisor and I decided we would keep borrowing the same book as long as I could keep finding different libraries that could lend it, or if I had to borrow it again from the same library that they didn't mind. We did not come up with a hard and fast rule on how many times we would think is too much and left it up to the lending libraries. As long as we could find a library still willing to lend it we would keep fulfilling the request (within reason. We might stop borrowing a book if the number of times became absurd. )
As for how to phrase a 3 time limit... How about something along the lines of "Due to restrictions on the part of lending libraries I am unable to borrow the same book for the same patron more than 3 times. Please be advised that this is the third time you have borrowed this book and I will be unable to bring it in again."

Good morning all,
We're a large public library and would like to limit the number of times
we borrow the same book for the same patron to 3 times. I'm trying to
come up with a brief statement to put on the third time borrowed book
telling the patron that this is the last time we'll borrow for them
without sounding too negative. Any ideas?
I've struggled with doing this as it doesn't happen that often so it's
not a written policy, but I feel 3 times is enough. Has anyone come up
with a good explanation without going into a lot of detail.
I'd also be interested to know how many of you impose a limit like this.
