We used to charge faculty through their department research accounts so the faculty themselves never really felt the sting. Each department would just transfer money into our ILL income account, based on how many monthly requests were made by their faculty.
We asked $5/article and charged an extra $0.20/page after 20 pages.
We did not charge for book delivery.

We are currently considering/entertaining the notion of providing a "fee-based" document delivery service for faculty only. I would like to hear from those of you that provide this service for a fee. Once I know who you are, I will check out your policies online.
