There's a sample reciprocal agreement in Appendix D of Interlibrary Loan and Document Delivery: Best Practices for Operating and Managing Interlibrary Loan Services in All Libraries, by Lee Andrew Hilyer. I'd be happy to send you a copy if you don't have the book.
