[ILL-L] Form

We have a form that covers all formats - it is a full 8x10 page and running
down the left, almost in the margin, are the formats like book, CD, DVD, MF,
Copy, etc.  Then to the right of that are blanks for author, title etc.
Below that is a separate place for Article requests with blanks for
Periodical title, article ti and au, vol, #, date, pgs, etc.  Then at the
bottom we have space for staff to check off date of request, receipt,
renewals, etc.  If I can get an electronic version I will email it to you,
It works very well for us, I have been rerfining it over a period of about
10 yrs.  You can contact me off list if you need to.
