I am attaching OHSU Library's ETD permission form.  Because OHSU has a
heavy focus on research, many of our authors have concerns about posting
their research online due to copyright issues concerning potential
publication, as well as time-sensitive research involving patents.  I
would suspect our form has more detail than most academic institutions
need.

Due to a recent upsurge in ILL requests for our institution's theses,
we're investigating making them available online.

 

We know we need some kind of permission/release form, signed by the
thesis author in order to make them available online and/or provide
electronic copies via ILL.  Can anyone who is currently doing this point
me toward a sample copy they use at their institution?

 

I'd also welcome any comments/warnings/insights from any libraries out
there who have made their theses electronically available.
