W won't fill the forms out for people. If they aren't filled out correctly and adequately and signed, one form per request, we send them back to the patron. We might email or call and tell the person that we are holding them until he/she can come back in to finish filling them out. That includes faculty. We do have online forms that can be used.

Well, it saves THEM time. I think I''ve already told the story of the patron who requested 126 items simultaneously--but only filled out one form. When I explained we needed one form per request, he told me, "YOU fill them out." (So I did.)

So here is one for you:
I just got a request from a patron who used one request form for five different books. They listed all the book titles in the title line of the form and just separated them using commas. Brilliant!
