[ILL-L] Responsibility for lost in transit items

Yes, the fact that the borrowing/requesting library is responsible for borrowed items lost in transit has long been part of the US ILL Code.  The current link for the code is http://www.ala.org/ala/mgrps/divs/rusa/resources/guidelines/interlibrary.cfm.  This is a 2008 revision, approved by RUSA in January 2009.  It contains no substantive changes but is stylistically different (more streamlined), according to current RUSA guidelines for writing guidelines.  The current section number for this provision is 4.8.  Section 4.9 extends the concept to user-initiated transactions. Collective wisdom needed -
What is the accepted protocol if a book is lost in the mail? In this case the lender sent the item to the borrower and the item never appeared.

Just for the record what would be the protocol if the borrower was sending it back to the lender and the item never appeared.
