is lost in transit.

Collective wisdom needed -
What is the accepted protocol if a book is lost in the mail? In this
case the lender sent the item to the borrower and the item never
appeared.
Just for the record what would be the protocol if the borrower was
sending it back to the lender and the item never appeared.
