[ILL-L] Doc Delivery, affiliated patrons, and copyright

Current situation: We are an academic health sciences library that also
serves the university's affiliated teaching hospital. This hospital is
not university owned and therefore does not have access to many of our
online journals (though some are available through shared licenses).
Concerning the journals that the hospital does not have access to, how do
other libraries out there handle copyright for these requests? Does it
"count" towards the rule of 5? Or would it be considered the same as
document delivery from out print based collection where that rule does not
apply. Should this group be considered a lending request or does their
legal affiliation make them "just another one of your patrons?"
