Actually I encountered a library that went beyond that. I couldn't understand why so many libraries had refused to loan a score one of our professors had requested, since the online catalogs I checked indicated it circulated, and the refusals were all "No - Unspecified" or "No - Policy problem." Finally we got the score. It was very thin, but two feet tall (actually I'm exaggerating--I measured and it was 23 1/2 inches tall) and nearly as broad. The library had had to construct a home-made mailing container out of several pieces of cardboard.

In response to an email about their ILL of AV policy, a library replied to me:
XYZ Library's "lending policy is if it's checked in and can be squeezed into a mailing container, we loan it."
That simplifies the Policies Directory greatly.
