[ILL-L] damaged item charges

We don't typically charge for damaged items unless they are beyond repair
and have to be replaced.  We do our repairs in house and this doesn't come
up often enough for us to sort out how to charge for it.

When an items is lost, we charge the actual cost to replace the item (we'll
check Amazon, Barnes & Noble and Baker & Taylor and take the cheapest price)
plus a $15 processing fee.

    We also check with Technical Services to determine the cost of repairing
the item or if it needs to be replaced.
The average repair cost is $25.00.

We charge a damage fee rather than replacement fee when only the binding has
been damaged and the book can be rebound. We check with our Technical
Services department to determine what the rebinding will cost and charge
that amount. It's usually under $20.

I'm working on a lost/damage charges document and would love some feedback
on what you do at your institutions.  Do you charge for repairs, as opposed
to charging a flat replacement fee?  How do you figure a fair amount to
charge for repair?
