[ILL-L] damaged item charges

Apologies for cross-posting.

I'm working on a lost/damage charges document and would love some 
feedback on what you do at your institutions.  Do you charge for 
repairs, as opposed to charging a flat replacement fee?  How do you 
figure a fair amount to charge for repair? 

Any insights would be appreciated.  Thanks.
