[ILL-L] incorrect citations in databases

I usually click on the button to "leave a comment"; whether it does any good is another matter.
Actually, I've been impressed by the accuracy of database citations; I generally find about three or four errors in them per year, but I frequently get three or four incorrect citations in a day from patrons (often the same patron). It's not always the patron's fault--often a patron submits a request, I take one look at the citation and think "That's wrong" and it is, but when I check the "source of citation" I find the patron copied it correctly. (The Web is the worst offender, of course, but I find a fairly high percentage of citations in published books and scholarly articles are wrong.)
Sorry--even when I have something positive to say, I can't resist turning it into a vent...

I'm finding more and more citations incorrect in our databases.
95% of our requests are directly from the databases (when they come through another way I always double check them through a search first) and more and more I'm getting "not found as cited" as a reason for not lending.
Granted, sometimes this is because a lender has just chosen the wrong radio button (and it automatically goes as unfilled even though in my borrowing comments I ask that you conditional not found as cited grumble grumble), but sometimes it really IS wrong.
Who do I contact about this? Can anyone tell me why this happens so often? Is it careless database entry or that the publisher changes things last minute?
It is making me grumpy.
