It's my understanding that this is indeed an ILLiad issue. There's a
problem when an ILLiad lender submits the 'shipped' update to OCLC and
it doesn't really go through. So, from the lender's end, it looks like
they have properly updated the request, but the OCLC status hasn't
changed. Some ILLiad users have recognized this and they look out for
these problem updates & manually update them to 'shipped' in WorldCat
Resource Sharing. We don't use ILLiad here, so I don't know if there's
any kind of error message or other notification when the 'shipped'
update doesn't go through.
To help with fixing the problem, the lender will need to contact OCLC
support (or maybe ILLiad support?) with the affected ILL numbers. I
heard from one lender that OCLC told them that the problem was caused by
something in the borrower's (my) profile, though we're not sure what
'profile' they were referring to, and I haven't heard anything else
about it for a couple of months. If there's a setting or field that I
can change, I'd love to know!
In the meantime, there are a few lender symbols that I took out of my
custom holdings groups since we were getting frequent duplicates because
requests aged from them.

We are seeing more and more of this: items shipped to us by
libraries that allowed a request to the next lender. We then end up with
duplicate copies --- wasted staff time and postage at both ends.
There are some libraries which we notice follow this pattern
with frequency. Others are intermittent. Should I bring it to the
attention of the libraries which allow the aging and then ship anyway?
Or is this due to some disconnect between ILLiad or Clio and WorldCat
that they can't help? (We do not use ILLiad or Clio here.)
