That's what we do here, but I know there are some libraries who do 
not loan the CDs out at all.  Some shelve them separately at the 
Reserve desk, others shelve them in a special area, etc. For what 
it's worth, as long as we have the CD in the book, we loan it out and 
note that on the paperwork.  On the rare occasion the book comes back 
without the CD, we bill for that.
On the borrowing side, I *do* appreciate it when a lender conditions 
us that they cannot loan the CD, and in those cases, I contact our 
patron and ask if they can still use the loan (it's quite often for 
computer language manuals and that sort of thing, so the CD can be a 
very important part of the loan).  However, we do also get books 
without the CD and no condition, and we just check the book out to 
the patron and hope they're happy to have the book. :)
