[ILL-L] how long before you assume an item is lost in the mail?

Great timing on the question--today is Bill Day at my institution. :)

We generally wait 3-4 weeks after the due date and recall the book.  3-4 weeks after the recall we send a bill.  We try to talk to the borrowing library and find out what's going on, and can adjust this schedule if needed.
