[ILL-L] Help: Does anyone have Journal of Theoretical Medicine in print?

Hello,

We're looking for:

Bajzer, et al
Journal of Theoretical Medicine, 2:  307-15, 2000

All the online versions are missing pages.  I've informed Taylor and 
Francis, but haven't heard back that they've fixed the online 
version.  I've checked OCLC holdings, but it's difficult to see if 
anyone actually has this in paper.  Does anyone have this in paper 
that can make us a copy?  We can send this via OCLC, fax, or email, 
and can pay up to $50.  Our OCLC code is LUP.
